    I need to change the contact form as shown in the jpeg attached. can you please send me the custom CSS that I need to use.

    Hi Jamal,

    Paste the following CSS code in your Custom CSS area to have these inputs in a full-columned view:

    .contact-form .col-sm-6 {
    width: 100%;
}
