Hi Marcin,
1. Install Ninja Forms plugin and set up a contact form, it will show there, or if you want use the default Kalium contact form.
2. You'll have to edit the row background, for more please follow the video I recorded: https://d.pr/v/ReZbgy
3. Edit your homepage and scroll down to the Parameters and Options part of the site, now follow my instructions on the attached image:
