If you have imported Architecture demo you will get the same design, please check if you have any other contact page on your Pages section. I thought that you wanted Version 2 and that's why i send to you the codes.

We haven't used Ninja Forms plugin for the Architecture demo, we used that plugin only for the contact page of the Photography demo.
I can see that you are trying to achieve the look like on the Contact page of the Photography demo.

In my iphone 7 nothing is similar. The information disappears on the left side. And the boxes with color do not carry.
